Appointment Information
Evaluations related to workplace injuries or critical incidents, disability, medical or administrative leave, and other employment-related concerns are conducted in a confidential clinical setting. Your evaluation and treatment remain private and are not automatically shared with your department, supervisor, employer, or other outside party. When documentation is requested for leave, disability, accommodations, workers’ compensation, or another employment-related purpose, we limit the information disclosed to what is authorized by you and/or required or permitted by applicable law. Our role is to provide an independent clinical evaluation and recommendations based on your mental health needs—not to represent your department or employer. Seeking care does not mean giving your employer unrestricted access to your mental health record.
Cancellation Policy
48-Hour Cancellation & Rescheduling Policy To ensure appointment availability for all clients, we require at least 48 hours' notice for cancellations or requests to reschedule. Appointments that are canceled or rescheduled with less than 48 hours' notice, as well as missed appointments (no-shows), will be charged $100 fee as outlined in our financial policies. Providing advance notice allows us to offer the appointment time to another client who may be waiting for care. If an unexpected emergency prevents you from attending your appointment, please contact our office as soon as possible so we can review your situation. We appreciate your understanding and cooperation in helping us provide timely access to care for all of our clients.
