Insurance coverage varies by plan, carrier, and individual benefits. While we are in-network with several insurance providers, coverage decisions are ultimately determined by your insurance plan—not by our practice.

Please note that insurance verification is not a guarantee of payment. Patients are responsible for understanding their individual plan coverage and any associated costs.
Out-of-Network & Self-Pay Options
If your insurance plan is out-of-network, or if you choose not to use insurance, self-pay options are available.

Initial Psychiatric Evaluation: $300
Follow Up: $120-$200
Therapy: $120
Reimbursement for out-of-network services is not guaranteed, and we do not provide superbills for all services. Self-pay rates and expectations can be reviewed prior to scheduling.
Craig Tiger Act (First Responder Mental Health Coverage)
Under Arizona law, including A.R.S. § 38-672 and § 38-673, commonly referred to as the Craig Tiger Act, first responders, including law enforcement may be eligible for mental health support services following qualifying duty-related traumatic exposures. This may include access to counseling services for a defined period, often up to 36 sessions within a 12-month timeframe, depending on eligibility and departmental processes.
This legislation recognizes the impact of cumulative and acute trauma in first responders and is designed to improve access to timely, appropriate mental health care without requiring the use of personal leave in certain circumstances.
Our team has experience working with first responders navigating care under this framework and provides thorough psychiatric evaluation, treatment, and clinically appropriate documentation when indicated.
Payment & Financial Responsibility​

Copays, deductibles, and coinsurance are the patient’s responsibility

Payment is due at the time of service unless otherwise arranged

Any remaining balance not covered by insurance will be billed directly to the patient
Missed appointments and late cancellations are subject to a $75 fee.
Services Not Typically Covered by Insurance
Insurance plans often do not cover certain services, including but not limited to:

Extended or non-standard appointments

Completion of forms (school, disability, legal, workplace, or other documentation)

Care coordination outside of scheduled visits

No show or late cancellation fees

Certain integrative or functional psychiatry–informed services, supplements, and compounded medication
